Started in April 2019, CREST is a community outreach team which aims to serve as a community safety network for people with/at-risk of dementia, depression and/or other mental health conditions. Through organising events and providing education, CREST reaches out to residents and caregivers to raise awareness and promote early recognition of signs and symptoms of dementia and other mental health conditions.
It also provides information and support to clients and their caregivers through emotional support, service linkage to the appropriate resources, follow-up sessions and support network.
CREST also trains volunteers to be Dementia Champions/ Befrienders and provides facilitation in the development of a dementia-friendly community in Woodlands.
Eligibility Criteria:
· For older adults with a formal diagnosis or suspected of dementia, depression and/or other mental health conditions
· Caregivers who need support
· Residing in our service boundary: Ang Mo Kio (Yio Chu Kang), Woodlands, Sembawang, Gambas, Canberra
