CREST (Community Resource Engagement and Support Team)

AWWA CREST is a community-based programme that aims to serve as a community safety network for older adults diagnosed/suspected of having dementia, depression and/or other mental health conditions.

Through outreach, public education and programmes, CREST reaches out to residents, caregivers and the community to raise awareness of dementia and other mental health conditions.

Through casework follow-up & monitoring, CREST provides information and education, emotional support, as well as service linkages to appropriate resources.

We also facilitate the development of dementia-friendly communities in Woodlands and Yio Chu Kang.

Eligibility Criteria:

  • Older adults diagnosed / suspected / at-risk of dementia, depression and/or other mental health conditions
  • Caregivers who need support
  • Residing in our service boundary (see below)

Constituencies of:

      • Canberra
      • Sembawang Central
      • Sembawang West
      • Woodlands
      • Yio Chu Kang

Key activities include:

  •  Events, psychoeducational talks and workshops relating to dementia, depression, and/or other mental health conditions
  • Mood and memory screening
 
 
  • Individualised casework follow-up through home visits
  • Collaborations with community partners to coordinate dementia or mental health training and education in the region
