CREST is an outreach team that focuses on raising public awareness of mental health conditions and dementia, promotes early recognition of at-risk individuals, provides emotional support to individuals and their caregivers and links individuals to relevant health and social care services when necessary. 

Eligibility Criteria:

• Individuals identified or at risk of having mental health conditions and dementia
• Caregivers of identified individuals who may be susceptible to or facing caregiver stress
• Living within service boundaries in the constituencies of:
  • Canberra
  • Sembawang Central
  • Sembawang West
  • Woodlands
  • Yio Chu Kang
