Community Resource, Engagement and Support Team (CREST)
The Community Resource Engagement and Support Team (CREST) serves as a mental health first-stop touchpoint. CREST promotes early identification of at-risk individuals and provides psychosocial intervention to support those with mental health needs and their caregivers in the community.
Eligibility Criteria:
- ndividual who is at risk of or diagnosed with a mental health condition and/or dementia
- Caregiver who supports someone at risk of or diagnosed with a mental health condition and/or dementia, and/or is experiencing caregiver stress.
- Live within the Ang Mo Kio service region.
Key activities include:
- Events, psychoeducational talks and workshops relating to mental health conditions and dementia
- Individualised casework & monitoring through home visits
- Information & referral
- Mood and memory screening
- Network and collaborate with community partners to enhance support for those with mental health conditions and dementia
